1. Print & FAX completed Spiritual Father’s Recommendation Form – Click here.
2. Direct individuals to reputable resources & agencies.
3. Do not encourage the individual you advise to contact you directly.
4. Make all contacts through the SVRM. 
5. Do not diagnose or prescribe treatment via phone or e-mail.
6. Complete online survey each time a contact is made.
7. Maintain strict confidentiality and professionalism.
