Middle school youth, boys and girls, grades 5 (or going to 6), 6, 7, 8 and 9 (or going to 10)
(Grade = grade of child when he/she enters school in the Fall of 2019.)

Cost: $140 per attendee (Sunday, June 30, 2019 at 11:59 PM)
Late Fee: $150 per attendee (Sunday, July 14, 2019 at 11:59 PM)

The first due date to to send the sheet of attendance is Sunday, June 30, 2019 at 11:59 PM.
The second due date to to send the sheet of attendance is Sunday, July 14, 2019 at 11:59 PM.

| Attendance Sheet: |
|
The attendance sheet includes the names of the kids and the servants who will attend the convention. It just needs to state the church and the priest’s name.
Each church must send a servant to attend and be the leader for his/her group.
Please add the t-shirt size of each attendee on the attendance sheet for t-shirts will be included in the registration price
The attendance sheet is to be sumbitted before the deadline to attitudeeems@gmail.com.

| Rooming: |
|
Please indicate in the attendance sheet the rooming assignements. Each room will have 4 or 5 people. It is preferred to have a servant in each room.

|
Make your check payable to your church and submit it to your priest or the camp contact person.
|

|
Payment is through Venmo and it must be submitted by the deadline to the committee.
