Does Your Grace think that a career in medicine in the US is too demanding and that it could take away from other priorities (e.g. service)?

You need to decide for yourself what you want to do the rest of your life. This decision should be totally yours. If you decide you really want to do medicine, then you should strive to do so. I do not think there is anything in life that can take us away from loving God and serving Him. We can serve God at school, at work or at any other place. Being busy with school and studying is temporary. With some time management we can commit ourselves to serving the Lord and studying at the same time. I know many medical students who went through medical school and never left serving at Sunday school; nor as deacons except of course when they were on call or in the middle of exams. As the saying goes "When there is a will there is a way".
