(Grade = grade of child when he/she enters school in the Fall of 2014.)

The due date to pay and to send the sheet of attendance is the evening of Sunday, July 20, 2014.

| Attendance Sheet: |
|
The attendance sheet includes the names of the kids and the servants who will attend the convention. This sheet must be signed by the Priest of the church.
Each church must send a servant to attend and be the leader for his/her group.
The attendance sheet is to be sumbitted before the deadline to Fr. Tadros Hirmina.

|
Please indicate in the attendance sheet the rooming assignements. Each room will have 4 or 5 people. It is preferred to have a servant in each room.

Make your check payable to your church and submit it to your priest or the camp contact person.
